HEALTH SERVICES
Sturgeon Bay School District has partnered with Door County Medical Center to provide nursing care for your child(ren) during the school day. The role of the nurse is to collaborate with staff, students, parents, and community resources to promote the well-being of your school aged children to maximize the potential for learning.
Functions:

To identify and assess student health concerns which may impact learning and intervene as appropriate
-
To develop health plans for students with disabilities and/or health conditions that interfere with learning

To provide health counseling to students, families and district staff
-
To initiate and follow up on referrals to health care providers
-
To train staff providing delegated nursing acts
-
To maintain confidential student health records
-
To act as a school/community/health care provider liaison
-
To collaborate with school staff and community agencies regarding student immunizations
-
To consult and collaborate with student services and other school staff informally and during building consultation team meetings
-
To participate in special education evaluations and individual education plan meetings for students who have significant health concerns
-
To provide input for the development of school district health policies
-
To develop and implement protocols for emergency care, illness/injury intervention and medication administration

To serve on district and community committees and coalitions related to health and wellness
