Instructions for New Employee Forms

Please print and submit only the pages of the forms that contain your information. You do not need to print the instruction pages or the exemption worksheets. Please fax, mail, or deliver these documents to the Business Office (located in the district office). Do not email them as they contain your private information.

Background Check Form

A background check form is required for all new hires and volunteers. This is sometimes done during the interview process. You must have a valid email address to complete the background check process. Please include that on your form. After you complete this form, please circle the question about previous residency, sign it and return it to the Business office in a sealed envelope. No one other than the Business Office will or should see your completed background check form.

W-4 Federal Withholding Form

This form can also be used by current district staff to change their exemptions already on file. Complete this page, sign, and forward to the Business Office. (It is only necessary to submit page 1 of this form.) Do not email.

State WT-4 Withholding Form

This form can also be used by current district staff that wish to change their exemptions.

I-9 Employment Verification Eligibility

Please see the list of personal documentation new staff is required to submit on page 9 of this form as proof of citizenship/work eligibility. New staff must submit the documentation with your employment papers before a check can be issued to you.

Direct Deposit Form

Direct deposit is required of all district employees. It may be necessary to first issue one regular check to the employee before the direct deposit information will be initialized. This form can also be used by current district staff that wish to change their direct deposit information on file.
