This form should be filled in completely, signed, and returned to the Business Office Assistant in a sealed envelope. (Please make sure to circle the residency question on the form.)

Mandatory Reporting Requirements

After you watch the video, you will be prompted to submit an email address to send confirmation of your completion. Please enter kgordon@sturbay.k12.wi.us. Kim from the Business Office will print your completion certificate that will be emailed to you and also retained in the Business Office as proof that you have completed this requirement.

WECAN

Sturgeon Bay School District uses WECAN for all applications except coaches and subs (e.g. sub teachers, sub associates, subs for the lunch program, etc.). The SUB application is posted below
